Dana Bonezzi is a graduate of Northwestern Ohio Universities College of Medicine and Ohio State Universities.

Debra Fast graduated both from Ohio University College of Osteopathic Medicine and the University of Akron.

Kathleen Fearon holds degrees from Ohio University College of Osteopathic Medicine, West Virginia University, and St. Vincent College

 

 

Besides offering comprehensive care we also provide Monday - Thursday Walk-in Clinic, prescription hot line, skin care, laser hair removal, and massage therapy, and Reliv, a high potency nutritional suppliment. Our regular office hours are: Monday 9:00 am – 5:00pm, Tuesday – Thursday 8:00am – 4:00pm, and Friday 7:00am – 3:00pm.

Open Scheduling

We encourage our patients to make use of our open scheduling hours for minor problems and illness such as sore throats, colds and flu.  These open scheduling hours are: Monday through Thursday from 8:am - 8:30 am. No appointment is necessary and new patients are welcome.

 Appointments

 

We take appointments and make every effort to see patients as quickly as possible. We try to follow a timely schedule; however, sometimes it becomes necessary to divert attention to severely ill patients that present with life threatening emergencies. We ask for your understanding when this happens. We will keep you informed of delays so that you may reschedule your appointment if necessary.

We follow our appointment schedule and not the order of sign-ins. At times we may call patients out of order. These patients may only need to see the nurse, or have blood drawn. If you are unable to keep an appointment, please call to cancel or reschedule your appointment as early as possible. This will be helpful to other patients.

Your Office Visit

For your first Visit with the Doctor you will need to fill out a medical history form. Please bring with you on every visit, a list of all the medicines or drugs that for any reason you are currently taking. The dosage of each will also need to be listed. Please bring a list of all the medicines that have caused you an allergic reaction. This information will be used by the Doctor to help provide comprehensive care.

Fees, Billing and Insurance

Our office is happy to fill out disability, handicap and other related forms. It may take up to five working days to complete. If you have not been seen for your disability in over six months, an evaluation may be required. Fee for paperwork - there will be a fee charged for all paperwork completed.

We accept many insurances including HMOs, PPOs, POSs, and Medicare assignment.

Please ask our receptionist if we participate in your plan.

 

 If you are coming to our office for the first time please note the following,

ˇ       We will need a copy of your insurance information and your driver's license.

ˇ       You will be responsible to pay your deductible and/or co-payment at the time of service.

ˇ       Please let our billing office know if you have any circumstances that need special requirements.

ˇ       Payment is expected at the time of service.

ˇ       We accept cash, check, Visa and Mastercard for your convenience.

Prescriptions & Refills

Script Hotline! Some prescriptions can be refilled by calling the office. Please provide your pharmacy phone number and the name and dose of the medication. Refills are not considered an emergency, and will not be handled after regular office hours. No controlled substances will be filled after hours. Prescriptions require a 24 hour notice. When calling the office you will be transferred to our prescription hotline.

General Comments

Please inform our office of any changes in your last name, address, phone number or insurance information. This office is a smoke-free environment. All of us at Comprehensive Internal Medicine look forward to caring for you, and we welcome you to our patient family.

Phone Nurse

When you call the office and need to speak with a nurse regarding a concern you will be transferred to our phone nurse.

After Hours

If you have an emergency call the answering service at 330-263-8500. If you have a life-threatening emergency you are encouraged to use the nearest emergency room.

 

 

We here at Comprehensive Internal Medicine now have available the Reliv nutritional supplement. Many of our patients are using this product and have found positive results, if taken as recommended.

A trial was conducted in 2001 with some of our current patients, as well as the CIM physicians. Using this scale, each symptom was given an average rating:
